The Polis Center is working with the Richard M. Fairbanks School of Public Health (FSPH) and the Marion County Public Health Department on the five-year Diabetes Impact Project: Indianapolis Neighborhoods (DIP-IN), a pilot undertaking funded by Lilly Global Health to assess current approaches of diabetes intervention and recommend new methods. Polis used SAVI to help the project partners select three Indianapolis neighborhoods with significant health disparities, high rates of diabetes, demographic factors, and highly-engaged community members and organizations: the Coalition of Northeast Neighborhoods, Near West, and Northwest.

The plan focuses on three main goals surrounding diabetes: increase screening-seeking behavior for those at high-risk of diabetes, improve access and continuity of care for people with diabetes, and foster an environment in the communities (both physical and social) that supports diabetes control and prevention. While this project focuses on diabetes, it also focuses on improving general quality of life in these three larger communities. You can learn more at diabetes.iupui.edu.
